We issue COAs on a case-by-case basis. Please enter the product's name for which you wish to receive a COA. Please include the invoice number or order ID number. If you have a specific lot number in the front of our label(s), please include that number in this form.

Please note it takes our compliance department 24-72 business hours to issue a COA when the information submitted is complete. Please verify your email folders for the PDF document(s).
